Forms
You will need either
Acrobat
Reader or Microsoft Word® to
open and/or edit these forms.
IMPORTANT NOTE ABOUT FORMS: you cannot type into most
of these .pdf forms and then just save them without losing the
information. You may, however, save a copy of a completed for IF
you are able to print to file.
To save completed forms to a file, you need to
download two parts of the same program.
1 and 2. Install these (they
are free) and once you have completed your form, then hit "print".
Choose the PDF995 printer instead of your regular printer. It will save
a copy of the completed forms onto your hard drive. (Windows only) Once
you have saved a copy, then print a hard copy to turn in or for your
records. For a shareware Mac program, click
HERE (registration
is $20).
Please email Mrs. Klaus via First
Class for the password to protected forms.
